Dear Partners Health Plan Network Provider,


I’m delighted to inform you that Steven Merahn, MD has joined Partnerships Solutions as Medical Director supporting both Partners Health Plan (PHP) and Care Design NY. As you may remember, Partnership Solutions is a new Managed Services Organization (MSO) exclusively dedicated to serving individuals with Intellectual and Developmental Disabilities (IDD) and their families, containing nonprofits PHP and Care Design NY.  Dr. Merahn will be taking the lead on our clinical policy and program development, as well as serving as a resource on patient experience and with our healthcare and care management operations.  Dr. Stephan Deutsch will continue to serve the PHP community as a Physician-Consultant.

 

Dr. Merahn has served in a variety of physician-executive leadership roles in his career, with unique experience redesigning care delivery and clinical operations for person-centered care, and systems-based practice with a special focus on clinical integration for special needs communities.

 

Dr. Merahn comes to us having most recently served as the Chief Medical Officer (CMO) for a national autism services provider serving almost 3,000 families; prior to that he was the CMO for US Medical Management, a multi-state physician network providing home-based primary care to medically complex and homebound elderly and disabled patients, where he led the transformation to value-based care, becoming a top 5 performing accountable care organization (ACO) in the country.  As Senior Vice President and Senior Medical Director at Aetna’s ActiveHealth Management Unit, he led clinical program teams focused on accountable care, providing clinical analytics, care management, and patient engagement initiatives for provider networks, health systems, and employer plan sponsors.

 

A Fellow of the American Academy of Pediatrics, he is a graduate of the Albert Einstein College of Medicine in the Bronx and completed his training at Montefiore Medical Center and the Bronx Municipal Hospital Center.  Dr. Merahn’s expertise and thought leadership have led him to national speaking engagements covering topics such as healthcare transformation and health equity. He is the author of Care Evolution: Essays on Health Care as a Social Imperative, which advocates for health as a social imperative and for healthcare as a fundamentally human endeavor. He serves as the only physician on the Autism Commission on Quality (ACQ) and is a member of the Primary Care Subcommittee of the American Academy of Pediatrics Section on Developmental and Behavioral Pediatrics (SODBP).
