About us

Care Design NY is a Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for 27,000 individuals with Intellectual and Developmental Disabilities (IDD) in 30 New York State counties, so they may lead a quality life. Care Design NY utilizes a person-centered and holistic service planning approach focused on desired outcomes by coordinating and delivering integrated supports and services with individual, family and provider input.     On April 3, Care Design NY joined advocates in Albany to urge legislators to invest in IDD services. Final negotiations were underway to establish funding priorities for the State’s FY 2024 – 2025 budget. Advocates’ voices rang out, “Invest in Me!” Members of the Senate and Assembly were on hand to show their support.
